Consent Form for Collection, Use and Disclosure of Personal Data

บริษัท กรุงเทพดุสิตเวชการ จำกัด (มหาชน) (“บริษัทฯ”) ให้ความสำคัญต่อการคุ้มครองข้อมูลส่วนบุคคลของท่าน และดำเนินการให้สอดคล้องกับพระราชบัญญัติคุ้มครองข้อมูลส่วนบุคคล พ.ศ. 2562 รวมถึงกฎหมายและประกาศที่เกี่ยวข้อง บริษัทฯ จึงขอความยินยอมจากท่านในการเก็บรวบรวม ใช้ และ/หรือเปิดเผยข้อมูลส่วนบุคคลที่ท่านให้ไว้แก่บริษัทฯ เพื่อวัตถุประสงค์ที่เกี่ยวข้องกับการจัดงานประชุมวิชาการ BDMS Academic Annual Meeting 2026

Bangkok Dusit Medical Services Public Company Limited (“the Company”) values your privacy and is committed to protecting your personal data in accordance with the Personal Data Protection Act B.E. 2562 (2019) of Thailand and other applicable laws and regulations. The Company hereby requests your consent to collect, use and/or disclose your personal data provided to the Company for purposes related to the organization of BDMS Academic Annual Meeting 2026.

บริษัทฯ จะเก็บรวบรวม ใช้ และจัดเก็บข้อมูลส่วนบุคคลของท่านเท่าที่จำเป็น ภายใต้วัตถุประสงค์ ขอบเขต และวิธีการที่ชอบด้วยกฎหมาย เป็นธรรม และโปร่งใส เพื่อการลงทะเบียน การยืนยันตัวตน การตรวจสอบการเข้าร่วมประชุม การให้บริการระหว่างงาน และการดำเนินการอื่นใดที่เกี่ยวข้องกับการจัดงานประชุมวิชาการ BDMS Academic Annual Meeting 2026

The Company shall collect, use and retain your personal data only as necessary, under lawful, fair and transparent purposes, scope and methods, for registration, identity verification, attendance verification, event services and other activities related to BDMS Academic Annual Meeting 2026.

วัตถุประสงค์ในการเก็บรวบรวม ใช้ และ/หรือเปิดเผยข้อมูลส่วนบุคคลของท่าน มีดังนี้

2.1เพื่อดำเนินการลงทะเบียน ตรวจสอบข้อมูลการเข้าร่วมประชุม และบริหารจัดการการเข้าร่วมงานทั้งในรูปแบบ Onsite และ Online
2.2เพื่อดำเนินการเกี่ยวกับการเก็บหน่วยคะแนนการศึกษาต่อเนื่องตามวิชาชีพ ตามหลักเกณฑ์ของหน่วยงานหรือสภาวิชาชีพที่เกี่ยวข้อง
2.3เพื่อวิเคราะห์ข้อมูลเชิงสถิติ ประเมินผล และพัฒนาการจัดงานในอนาคต
2.4เพื่อสื่อสาร ประชาสัมพันธ์ แจ้งข่าวสาร หรือเชิญชวนเข้าร่วมกิจกรรม การอบรม หรือการประชุมวิชาการที่เกี่ยวข้องในอนาคต
2.5สำหรับผู้เข้าร่วมงานแบบ Onsite บริษัทฯ และ/หรือผู้จัดงานอาจมีการบันทึกภาพนิ่ง ภาพเคลื่อนไหว วิดีโอ เสียง หรือสื่ออื่นใดระหว่างการจัดงาน เพื่อใช้ในการบันทึกกิจกรรม การรายงานผล การประชาสัมพันธ์ การเผยแพร่ผ่านสื่อของบริษัทฯ สื่อออนไลน์ เว็บไซต์ สื่อสังคมออนไลน์ เอกสารประชาสัมพันธ์ หรือช่องทางอื่นที่เกี่ยวข้องกับการจัดงาน โดยจะดำเนินการภายใต้ขอบเขตที่เหมาะสมและเป็นไปตามกฎหมายคุ้มครองข้อมูลส่วนบุคคล

The purposes of collecting, using and/or disclosing your personal data are as follows:

2.1To process registration, verify attendance information and manage participation in both Onsite and Online formats;
2.2To process continuing professional education credits in accordance with the criteria of relevant professional councils or authorities;
2.3To conduct statistical analysis, evaluation and improvement of future events;
2.4To communicate, provide information, publicize or invite participants to relevant future activities, training sessions or academic conferences;
2.5For Onsite participants, the Company and/or the event organizer may capture photographs, video recordings, audio recordings or other media during the event for event documentation, reporting, public relations and publication through the Company’s media channels, online platforms, websites, social media, promotional materials or other channels related to the event, within an appropriate scope and in compliance with applicable personal data protection laws.

บริษัทฯ จะเก็บรักษาข้อมูลส่วนบุคคลของท่านเป็นระยะเวลา 24 เดือน นับตั้งแต่วันที่สิ้นสุดการประชุมวิชาการ หรือเท่าที่จำเป็นตามวัตถุประสงค์ที่แจ้งไว้ หรือตามที่กฎหมายกำหนด เมื่อพ้นระยะเวลาดังกล่าว บริษัทฯ จะดำเนินการลบ ทำลาย หรือทำให้ข้อมูลไม่สามารถระบุตัวบุคคลได้ตามกระบวนการที่เหมาะสม

The Company shall retain your personal data for a period of 24 months from the completion date of the conference, or as long as necessary for the stated purposes or as required by law. Upon expiry of such period, the Company shall delete or anonymize the personal data through appropriate procedures.

บริษัทฯ จะไม่เปิดเผยข้อมูลส่วนบุคคลของท่านให้แก่บุคคลภายนอก เว้นแต่เป็นการเปิดเผยตามวัตถุประสงค์ที่ได้แจ้งไว้ ได้รับความยินยอมจากท่าน หรือเป็นกรณีที่กฎหมายอนุญาตหรือกำหนด ทั้งนี้ บริษัทฯ อาจเปิดเผยข้อมูลส่วนบุคคลของท่านให้แก่เจ้าหน้าที่ ผู้ให้บริการ ผู้ประมวลผลข้อมูล ผู้จัดงานร่วม หน่วยงานวิชาชีพ หรือบุคคลที่ได้รับมอบหมายซึ่งมีความจำเป็นต้องใช้ข้อมูลเพื่อดำเนินงานที่เกี่ยวข้องกับการประชุม โดยบริษัทฯ จะกำกับดูแลให้บุคคลดังกล่าวรักษาความลับของข้อมูลส่วนบุคคล และไม่นำข้อมูลไปใช้เพื่อวัตถุประสงค์อื่นนอกเหนือจากที่กำหนด

The Company shall not disclose your personal data to third parties unless such disclosure is made for the purposes notified herein, with your consent, or as permitted or required by law. The Company may disclose your personal data to staff, service providers, data processors, co-organizers, professional bodies or authorized persons who need to access such data for purposes related to the conference. The Company shall ensure that such persons maintain the confidentiality of the personal data and do not use it for purposes beyond the specified scope.

เจ้าของข้อมูลส่วนบุคคลมีสิทธิในการถอนความยินยอม ขอเข้าถึง ขอรับสำเนา ขอแก้ไข ขอให้ลบหรือทำลาย ขอให้ระงับการใช้ ขอให้โอนย้ายข้อมูล คัดค้านการประมวลผลข้อมูลส่วนบุคคล รวมถึงสิทธิอื่นตามที่กฎหมายกำหนด ทั้งนี้ การถอนความยินยอมจะไม่กระทบต่อการเก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคลที่ได้ดำเนินการไปแล้วโดยชอบด้วยกฎหมายก่อนการถอนความยินยอม

The data subject has the right to withdraw consent, request access, obtain a copy, request correction, deletion or destruction, request suspension of use, request data portability, object to the processing of personal data and exercise other rights as prescribed by law. Withdrawal of consent shall not affect any collection, use or disclosure of personal data lawfully conducted prior to such withdrawal.
